From one level of service to the next, you can count on the Case Management/Social Services Department at Wesley to be your guide. Our goal is to assist patients as they transition through the continuum of care, making it as safe, effective and efficient as possible. Our Case Management/Social Services team is composed of Case Managers, who are all Registered Nurses and masters-prepared Social Workers, each specially trained in assisting patients and patients' families with insurance beneficiary plans, discharge planning needs/arrangements, family or social issues and referrals. This team of specialized healthcare providers works diligently to maximize resources and coordinate benefits under your insurance or benefit plan. If you are without a benefit plan, they will help you in getting assistance and determining if you are eligible for a benefit program.
Should you have questions at any time about continued care, equipment or health services needed after your discharge from the hospital, or should you need assistance with a specific social issue, we encourage you speak with one of the Wesley Case Managers or a Social Worker.
