Medical Staff and Services
A physician's involvement in a patient's recovery does not stop with a referral to Wesley Home Care. In fact, physicians continue to work closely with the home care staff to monitor the progress of each home care patient. And as always, the physician remains in charge of the patient's overall care plan and will ultimately decide which services to be administered at home.

Skilled Nursing Care
You can trust our certified staff of RNs and LPNs to administer every home care plan and to report each patient's progress to his or her referring physician.

Physical, Occupational and Speech Therapy
Licensed therapists assess and develop rehabilitation plans for patients, in coordination with the referring physician. These therapists work to restore each patient to his or her maximum level of health. They may also offer instruction on the use of certain medical equipment, if necessary.

Certified Nurse Aide Care
Under the supervision of an RN, nurses' aides may provide assistance with activities such as walking or exercising.

Medical Social Services
A medical social worker aids the Wesley Home Care staff in planning a healthcare regimen. The social worker also ensures that the patient has the resources needed to make a quick recovery.
